Rafal Zielinski, aged 40, of Parkside Road in Reading, Berkshire, was killed following the two vehicle collision on Hartley Lane, at 4.40pm on Monday, October 24.
A silver Vauxhall Corsa and a Peugeot Boxer van were travelling in opposite directions when they collided head-on.
Sadly, Mr Zielinski, who was driving the Corsa, was pronounced dead at the scene.
The driver of the van and his passenger suffered minor injuries and were both taken to North Hampshire Hospital as a precaution.
Officers are supporting Mr Zielinski’s family at this time.
