THE acute care team at the Swan Medical Group doctors’ surgery in Petersfield made the top five in the UK final of the Practice Nursing Award, part of the GP National Awards 2021.

The ten-strong team, led by nurse practitioner Kat Graham, just missed out on winning the award that was entered by 288 nurse teams nationwide.

The team at Swan Surgery in Swan Street consists of two paramedics, a pharmacist and seven nurse practitioners.

Swan partner Dr Andrew Holden said: “We are incredibly proud of the team and its achievements. To be short-listed for the award shows how fortunate that we as a practice are to have them with us as we strive to provide the very best health services we can to our 21,000 patients.

“They provide a rapid access service, play a big part in providing long-term health services and a frailty service to our patients and those of a neighbouring practice, and a home-visiting service to more than 69,000 patients covering seven local practices.

“The team has gone above and beyond in the past year, single-handedly provided Covid vaccinations to our local care home residents and staff as well as housebound patients.”

Kat Graham has been central in setting up the local Covid-19 vaccination centre which has already delivered more than 100,000 jabs in the Festival Hall.

She said: “Day to day, the team has greatly supported our GPs, enabling them to see more complex conditions and has been running continually throughout the pandemic.”
